SAY SORRY FOR WHAT YOU DID!
In-Game Name
Who Banned You?

DO NOT APPLY FOR UNBAN IF YOU'RE JUST TEMPBANNED FOR A FEW DAYS! (If you are banned for over 4 days.. its ok)
